It comes as the coroner took the rare step of referring the case back to the Crown Prosecution Service (CPS) to consider corporate manslaughter or gross negligence charges.She told the Mirror: "It came as a shock because that was our seventh pre-inquest review hearing so we were expecting it to start in June.
