Ocak 2020'de tutulan tutanakta, Doku'nun POLNET ile TEMBİS Sağlık Bilgileri Sorgulama kayıtlarında 31 Aralık 2019'da saat 09.09'da Tunceli Devlet Hastanesine giriş kaydının bulunduğunun tespit edildiği belirtildi. Tunceli Cumhuriyet Başsavcılığınca, 27 Mart 2025'te bir firmaya bilgi ve belgelerin araştırılmasına yönelik yazıya verilen 16 Nisan 2025 tarihli cevapta, yeniden ve daha ayrıntılı bir inceleme gerçekleştirildiği bildirildi. POLNET kayıtlarının resmi kayıt olduğu ve hata ihtimali olmayacağının dikkate alındığı aktarılan yazıda, şunlar kaydedildi: "Gülistan Doku'ya dair herhangi bir vizit veya LOG bilgisine ulaşılamamıştır. Bu husus firmamızca tekrar incelenmiş ve o tarihte herhangi bir LOG bilgisi olmadığı görülmüştür. 31 Aralık 2019 ila 31 Ocak 2020 arasında diğer firma tarafından Tunceli Devlet Hastanesine HBYS sistemi hizmeti verilmekteydi. HBYS sisteminde yer alan işlemlere ilişkin LOG kaydının bulunması gerekliyken bu tarihlere ait herhangi bir LOG kaydının bulunmaması hayatın olağan akışına aykırı olup profesyonel ve detaylı bir çalışma ile özellikle bu tarih aralığındaki LOG kayıtlarının kasıtlı bir biçimde silindiği tarafımızca değerlendirilmiştir. İlgili LOG kayıtları ancak bu yazılımın ana kodlarına sahip ve bu yazılımı kullanmayı bilen profesyonel düzeyde yazılım bilgisi içeren şahıslar tarafından çeşitli doğrulamalar yapılarak kasıtlı ve yoğun bir şekilde emek harcayarak silinebilmektedir. Dolayısıyla silinen kayıtların geri getirilme ihtimali ve imkanı mevcut değildir. Sonuç olarak 31 Aralık 2019 tarihine ait sistem LOG'larının bulunmaması olağan bir durum olarak değerlendirilememekte, ilgili güne ait LOG kayıtlarının kasten ve yetkisiz teknik bir müdahaleyle silindiği anlaşılmaktadır." Tunceli Cumhuriyet Başsavcılığınca, diğer firmaya yazılan yazıda, bazı hususlara ilişkin anlaşılabilir ve net bilgiler verilmediğinin anlaşıldığı belirtildi. Yazıda, İl Emniyet Müdürlüğü tarafından POLNET sistemi üzerinden alınan kaydın resmi belge olduğuna işaret edilerek, "Bu kaydın POLNET sisteminde yer alıyor olmasının Gülistan Doku'nun kesin olarak 31 Aralık 2019'da saat 09.09'da Tunceli Devlet Hastanesine giriş kaydı yaptığının delili olduğu, bu hususta kesinlikle hata olmayacağının sabit olduğu, bu nedenle detaylı araştırma yapılarak Gülistan Doku'nun 31 Aralık 2019'da hastane giriş kaydının 7 Ocak 2020 ila 9 Ocak 2020 arasında kim, hangi kullanıcı tarafından silindiğinin titizlikle araştırılması" istendi. Belirtilen güne dair 09.00 ile 11.00 arasında hasta cihaz giriş kaydı yapan cihaz ve kullanıcı bilgilerinin istenildiği yazıda, Doku'nun hamileliği iddialarıyla ilgili ise şu bilgiler yer aldı: "EK'te gönderilen CD'deki bilgilerin kime ait olduğu, hastaya kim tarafından hangi işlem yapıldığı ve benzeri hususların anlaşılabilir olmadığı, örneğin bir tabloda Gülistan Doku_HGEBDET uzantısında 'Bulgular-Gebe-Plan-Çocukkey-Htkey-Fetuskalses-Hbkey-Pelvisanatomi-Pelvisaçıklık ve benzeri' başlıklar olduğu, bu başlıkların gebelikle ilgili olduğunun değerlendirildiği ve kalp sesi ölçümlerinin yapıldığı anlaşıldığı ancak bu bilgilerin neye dayanarak belirtildiği, neye ilişkin olduğunun, Gülistan Doku'nun gebelik durumunun olup olmadığının, gebeliğe ilişkin hastaneye müracaatının olup olmadığının anlaşılamadığı, şayet gebe ise doğum için gerekli süre düşünüldüğünde tarihlerin de uyumlu olmadığının anlaşıldığı, Gülistan Doku'nun hastane giriş tarihlerinin silinen kayıtlar da dahil olmak üzere tek tek ve fiziki evrak yazılarak Gülistan Doku'ya ne sebeple hangi işlemlerin yapıldığının araştırılarak gönderilmesi rica olunur." Doku hakkında hastanede kayıt olmadığına dair yazının kabul edilebilir olmadığı vurgulanan yazıda, "İlgili tarih ve saatte yapılan giriş kaydının kesinlikle silinmiş olduğu ve bunun kasıtlı olarak profesyonel ve detaylı bir çalışma sonucu silindiğinin, Gülistan Doku'ya ait 31 Aralık 2019 saat 09.09'da yapılan hasta kayıt işlemini sonraki tarihlerde silen kullanıcı bilgilerinin mutlaka titizlikle, detaylı bir şekilde araştırılarak gönderilmesi rica olunur." Tunceli Devlet Hastanesinin Tunceli Cumhuriyet Başsavcılığının 12 Ekim 2020 tarihli yazısına verdiği cevapta ise "Gülistan Doku'nun kayıtlarımız tetkikinde 31 Aralık 2019'da hastanemizde herhangi bir girişinin olmadığı tespit edilmiş olup, bugüne kadar tedavi olduğuna dair belgeleri sunulmuştur." ifadeleri kullanıldı. Sağlık Bakanlığı, Gülistan Doku soruşturması kapsamında hastane kayıtlarının silindiği iddiaları üzerine, konuyla ilgili soruşturma başlatıldığını ve müfettiş görevlendirildiğini bildirdi. Bakanlıktan konuya ilişkin yapılan açıklamada, "Tunceli'de bahse konu olaya ilişkin Bakanlığımızca soruşturma başlatılmış, konuyla ilgili müfettiş görevlendirilmiştir." ifadesi kullanıldı.
