Fresh mystery over Kurt Cobain's death as astonishing error is uncovered from aftermath of Nirvana frontman's suicide

Yet a mysterious new discrepancy has now been unearthed in the Nirvana frontman's official death records. Cobain was found dead aged just 27 with a shotgun wound to the head at his Seattle, Washington, home on April 8, 1994. The Seattle Police Department quickly ruled that he had taken his own life after investigators recovered a suicide note and heroin kit nearby. Cobain's death certificate described his fatal injury as a 'contact perforating shotgun wound to head.' This indicated that the blast entered and then exited his skull. However, his autopsy came to a different conclusion. It stated that Cobain died from a 'penetrating' wound, which means there was an entrance wound but no exit wound. After reviewing the records, expert forensic pathologist Mary Jumbelic told the Daily Mail that all the physical evidence outlined in the autopsy proves there is clearly an 'error' on the death certificate. This finding does not undermine any official suicide ruling. But the discrepancy is now raising fresh questions about how it is possible that the record of one of America's most scrutinized deaths has never been corrected or clarified. The death certificate was signed on April 9, 1994 – one day after Cobain's body was found. The autopsy report, which was prepared by Assistant Medical Examiner Dr Nikolas Hartshorne and approved by Chief Medical Examiner Dr Donald Reay, was finalized two months later in June 1994. Both reports detail a contact shotgun wound to the head. But they each give different forensic definitions for the exact type of injury caused. The death certificate stated that Cobain died from a 'contact perforating shotgun wound to head.' But the autopsy attributed Cobain's death to a 'contact penetrating shotgun wound to the head' – documenting an entrance wound through the roof of his mouth, but no corresponding exit wound. It also stated that approximately 30 lead pellets and a distorted plastic shotgun-shell wad had been recovered from his brain, indicating that the blast remained inside his skull. Meanwhile, the amendment section of the death certificate was left blank and never updated after the autopsy findings were released. Jumbelic told the Daily Mail that 'perforating' and 'penetrating' have distinct meanings when forensic pathologists describe firearm injuries, and that the use of 'perforating' on the death certificate and medical examiner's determination page was an error in forensic terminology. Jumbelic also identified a second terminology mistake within the External Evidence of Injury section of the autopsy. In its second paragraph, the report described a '1 1/2 inch gunshot wound entrance defect' – despite the weapon being a shotgun. 'In forensics, shotgun and gunshot are not interchangeable terms,' Jumbelic said. 'They reference distinct weapons. Gunshot wounds are from handguns or rifles, neither of which are relevant in this case.' The autopsy finalized in June 1994 documented an entrance wound with no exit and recorded that shotgun pellets and shell wadding were recovered from inside his head It attributed Cobain's death to a 'contact penetrating shotgun wound to the head (mouth)' and said the suicide determination was based on 'the scene and circumstances surrounding his death' The expert said the discrepancies appeared consistent with 'human or typist error and incomplete double-checking of information.' 'Every case really should be carefully worded and checked. The decedent deserves that,' the pathologist added. The timing of when the records were made could also explain how conflicting terminology entered the official documents. Jumbelic said a death certificate is typically signed after the physical examination of the body has been completed, while a final autopsy report can take months because pathologists must wait for ancillary testing such as toxicology. In Cobain's case, the death certificate was signed on April 9, one day after the scene investigation, while the completed autopsy report was signed in June. When asked about the discrepancies, a spokesperson for the Public Health – Seattle & King County told the Daily Mail: 'Per Washington State law, we cannot share or discuss investigative details or autopsy records with anyone beyond next of kin or their designees.' The Daily Mail has contacted the Washington State Department of Health for comment.
