The Seattle Police Department quickly ruled that he had taken his own life after investigators recovered a suicide note and heroin kit nearby. Cobain's death certificate described his fatal injury as a 'contact perforating shotgun wound to head.' This indicated that the blast entered and then exited his skull. Jumbelic also identified a second terminology mistake within the External Evidence of Injury section of the autopsy. In its second paragraph, the report described a '1 1/2 inch gunshot wound entrance defect' – despite the weapon being a shotgun. 'In forensics, shotgun and gunshot are not interchangeable terms,' Jumbelic said. 'They reference distinct weapons. Gunshot wounds are from handguns or rifles, neither of which are relevant in this case.' The autopsy finalized in June 1994 documented an entrance wound with no exit and recorded that shotgun pellets and shell wadding were recovered from inside his head It attributed Cobain's death to a 'contact penetrating shotgun wound to the head (mouth)' and said the suicide determination was based on 'the scene and circumstances surrounding his death' The expert said the discrepancies appeared consistent with 'human or typist error and incomplete double-checking of information.' 'Every case really should be carefully worded and checked. The decedent deserves that,' the pathologist added. The timing of when the records were made could also explain how conflicting terminology entered the official documents. Jumbelic said a death certificate is typically signed after the physical examination of the body has been completed, while a final autopsy report can take months because pathologists must wait for ancillary testing such as toxicology. In Cobain's case, the death certificate was signed on April 9, one day after the scene investigation, while the completed autopsy report was signed in June. When asked about the discrepancies, a spokesperson for the Public Health – Seattle & King County told the Daily Mail: 'Per Washington State law, we cannot share or discuss investigative details or autopsy records with anyone beyond next of kin or their designees.' The Daily Mail has contacted the Washington State Department of Health for comment.
